From April of 1964 to Oct. of 1972, 41 patients with splenomegaly were treated at the
Dept. of General Surgery of S.N.U.H. A clinical analysis was made and the results were as
follow.;
1. The peak age incidence was in 3rd decade and male predominated over female by the ratio
of 1.9:1.
2. The most frequent complaint was abnormal bleeding including hematemesis, epistaxis and
gum bleeding, and most of patients (60%) had symptoms less than 1 year.
3. Reasons- of splenomegaly were analyzed as- follows:
Portal hypertension 32 cases
Congenital- spherocytosis 2
Myeloid metaplasia of spleen 2
Chronic myeloid leukemia 2
Primary hypersplenism I
Hemangiosarcoma 1
Hepatoma I
4. On admission anemia was noticed in 70.9%, leukopenia in 63.4%, thrombocytopenia in
78.4% and pancytopenia in 27%.
5. Operations were. performed in 36 of 41 cases and they were splenorenal shunt in 21 cases,
splenectomy in 12 cases, portocaval shunt in 1 case and laparatomy in 2 cases.
6. Four out of 41 patients died, 3 in operated group and 1 in nonoperated group, and total
mortality was 9.8% and the operative mortality, 8.3%.
